Please make your class selection, then print out this form, complete it and send it to the address at the bottom of the form, or fax it to our phone / fax number 763-422-0729.  Make as many copies of the form as you need. One form per class and student please.  If you have any questions, please contact us by e-mail, phone or fax (e-mail is preferred and will generally receive a quicker response due to Vern's travel schedule). Class payment and refund information

   Tuition payment for the class(es) is due in full 30 days before the class start date. Refunds for canceled registration will be given until 14 days before the class start date.  Registrations canceled within 7-14 days will be assessed a 30% cancellation fee.  Registration canceled within 7 days of the class start date will not be refunded.  Non-refunded registration fees may be applied to future M.O.V. Training Services classes.  Class size is limited and registration will be on a first come first served basis. M.O.V. Training Services accepts Visa, MasterCard, Discover and American Express as payment of tuition. If you wish to use this service, click here!
